What Steps are Necessary to Apply for Accreditation?

The steps to accreditation through NBAOS are as follows:

Step 1: Complete the Accreditation Initial Application and submit the application and the $100 application fee to NBAOS.

Step 2: Once the application is received by NBAOS, the facility will be provided with an accreditation manual that contains the complete survey information required at the time of the on-site survey, along with sample forms.

Step 3: Once the facility has determined it meets each of the requirements and has all the required information in place, the facility will submit the Onsite Accreditation Application to NBAOS indicating the facility is prepared for the on-site survey.

Step 4: The on-site survey is an UNANNOUNCED survey. It will be conducted during the business hours reported in the application. Each office location is subject to an unannounced survey.

Step 5: The facility will be notified of their accreditation status within 30 calendar days following the survey.  Facilities with be accredited, placed on probation or notified that the facility does not meet the required standards established by CMS.

What are the Costs for Accreditation?

ACCREDITATION FEES

$100 Initial Application Fee

1. Facility may apply at any time
2. Facility will receive detailed listing of survey requirements

$1,500 On-Site Survey Fee

1. Facility will submit when prepared for on-site survey
2. Unannounced survey will be conducted
3. Note: Facilities in remote areas (more than two hours from a city with a population greater than 100,000) will be responsible for transportation, meals and accommodations necessary to conduct the survey.

$350 – Each Satellite Location Fee

1. Facility will submit when prepared for on-site survey at primary office location
2. Unannounced survey will be conducted
3. Surveyor travel expenses (transportation/meals/accommodations) are the responsibility of the facility when the surveyor travels more than 30 miles to the survey site from the primary office site.

$1,500 Re-Accreditation (Renewal) Fee

1. Required each three years
2. Unannounced survey will be conducted
3. Re-accreditation fees are subject to revision as warranted by NBAOS

$350 - Each Satellite Location Renewal Fee

1. Facility will submit when prepared for on-site survey at primary office location
2. Unannounced survey will be conducted at each office location
