Steps for Accreditation and Reaccreditation

The steps for accreditation through NBAOS are as follows:

Step 1: Complete the Accreditation Initial Application and submit the application and the $100 application fee to NBAOS.

Step 2: Once the application is received by NBAOS, the facility will be provided with an accreditation manual outlining all accreditation requirements in detail, information about the onsite survey and sample forms. Step 3: The facility will then gather all the information required to complete the onsite survey including preparing the facility, gathering documents, and organizing all of the required material outlined in the accreditation manual. This process may take a few weeks to many months depending on the facility's current business, operational and organizational arrangement.

Step 4: Once the facility has determined it meets each of the requirements and has all the required information in place, the facility will submit the Onsite Accreditation Application to NBAOS indicating the facility is prepared for the on-site survey. Please Note: Once this application is submitted with payment the window for the unannounced site survey visit is open.

Step 5 : The on-site survey is an UNANNOUNCED survey. It will be conducted during the business hours reported in the application. Each office location is subject to an unannounced survey.

Step 6 : The facility will be notified of their accreditation status within 30 calendar days following the survey.  Facilities with be accredited, placed on probation or notified that the facility does not meet the required standards established by CMS.

Fees for Accreditation and Reaccreditation

$100 Initial Application Fee

1. Facility may apply at any time
2. Facility will receive the onsite survey information and forms

$2,750 + Fees* On-Site Survey Fee (*plus all surveyor travel expenses)

1. Facility will submit the Onsite Accreditation Application
2. Unannounced survey will be conducted

$500 Each Satellite Location Fee

1. Facility will submit this fee when prepared for the on-site survey at primary office location
2. Unannounced survey will be conducted

Additional Expenses

1. Surveyor travel expenses - Transportation, meals, accommodations, etc. are the responsibility of the facility

$2,000 Reaccreditation (Renewal) Fee

1. Required each three years
2. Unannounced survey will be conducted
3. Re-accreditation fees are subject to revision as warranted by NBAOS

$500 Each Satellite Location Renewal Fee

1. Facility will submit this fee when prepared for the on-site survey at primary office location
2. Unannounced survey will be conducted at each office location
